Requesting Assistance

All patients will have the opportunity to speak with NUCF/UHO’s Patient Advocate. Upon initiation of care with the Practice, the Patient Advocate will begin the process of working with each person to determine what resources are needed and then strives to match those needs with public resources first and then NUCF, if necessary. If comfort items are available and desired, they can be provided at any time.

Please understand that in order to access the full scope of assistance which may be available, the Patient Advocate will need your Social Security number, income tax returns and other information. This information is required by outside agencies and NUCF to determine financial need.

Please recognize that NUCF and other resources are limited and that the Patient Advocate and Executive Director will work to ensure that funds are used in a judicious manner possible to benefit as many patients as possible.
